Mrs. Phyllis Lorraine Forbes Mizell, 83, of Lake City, FL died on Tuesday, November 29, 2011, at the Life Care Center of Orange Park, FL. She was born in Blakely, GA, February 24, 1928 and was the daughter of the late Hubert and Dorothy Forbes. She moved to Lake City in 1964 from Mt. Pleasant, FL. Having retired as a Registered Nurse from the Lake City Veteran's Administration Hospital, she loved spending time with her family, painting, gardening, wildlife, bird watching and was a loving mother, grandmother and great grandmother.
Survivors include one son: Tip Mizell, Lake City, FL; one daughter: Debbie Mizell Allen, Jacksonville, FL; four grandchildren: Trey Allen, Ashley Jackson, T. H. Mizell and Chet Mizell; five great grandchildren: Aubrey Mizell, Kylie Jackson, Brecken Jackson, Gauge Allen and Kyden Ferrandiz.
Funeral services will be conducted on Monday, December 5, 2011, at 2:00 P.M. at
Pleasant Grove United Methodist Church with Chaplain Jess McCrosky officiating. Visitation with the family will be one hour before the service, from 1:00 P.M. to 2:00 P.M.
